MEDICAL PLAN

Eligibility
This benefit requires 120 consecutive months of coverage in the plan immediately preceding the disability, SSA determination, physician's statement and application to Board of Trustees. Member must be unable to engage in any substantial gainful activity by reason of any medical or mental impairment that can be expected to result in death or last at least 12 months. This does not include disabilities that occurred while engaged in a felony, are self-inflicted, arise out of service in the armed forces or result in a member receiving workers compensation from outside the Electrical Industry.

Benefits
The member receives $600.00 per week for the first 26 weeks of disability. This is the short term benefit and would be exhausted before the long term benefit would begin. Following that, the member receives $325.00 per month until a maximum benefit of $20,500.00 (about 5 years) has been reached. Medical coverage is provided to the member and family through the Retirement Medical Funding Plan (see below).

PENSION PLAN

Eligibility
Member must be vested in the pension plan (5 years with at least 1000 Hours of Service in each year) instead of the 120 months rule as in the medical plan. All other eligibility requirements are the same as outlined above for the medical plan.

Benefits
Member receives a monthly benefit for life equal to the greater of the actual Accrual Service earned by the member or 20 times the benefit rate in effect at the time of the disability (minimum straight-life benefit of $1,000.00 for disabilities on or after 1/2009). Member may choose 50%, 75%, or 100% survivor annuity to provide benefits to a spouse if the member dies first; an actuarial reduction may apply. Benefits begin (and may be retroactive) the first of the month following the 26th week from the date of disability.

SUPPLEMENTAL PENSION & 401(K) PLAN

Eligibility
Member must be vested in the plan(s). All other eligibility requirements are the same as outlined above for the medical plan.

Benefits
Member may receive lump sum distribution of total account balance from each of the plans as a result of the disability by filing application with Board of Trustees.

RETIREMENT MEDICAL FUNDING PLAN

Eligibility
Member must be totally disabled as defined in the Pension Plan and has accumulated a minimum of 10 service credits under the RMFP.

Benefits
The Retirement Medical Funding Plan will make benefit payments to reduce the premium for coverage under the medical plan for the member and the family based on the number of actual service credits earned or 20, whichever is greater. This benefit begins on the date of disability as determined by the Trustees and continues until the member dies. If the member dies and the spouse survives, the benefit continues for the spouse until the spouse dies, the spouse remarries, or the spouse becomes eligible for coverage elsewhere, whichever occurs first.

OTHER BENEFITS TO CONSIDER

Social Security Disability benefits based on the years of employment and wages earned by the member.

National Electrical Benefit Fund (NEBF) benefits are similar to the benefits described above for the pension plan. A monthly benefit for life that is equal to the greater of the member's number of years of service or 20 times the benefit rate in effect at the time of the disability ($640.00 minimum as of 1/2002).
